Amigo Pay: Healthcare Payments Platform
A full-stack payment acquiring platform that digitizes financial operations for healthcare providers across Brazil.
Overview
Amigo Tech is a complete healthcare platform serving 6,000+ clinics, 50,000+ health professionals, and 18M patients in Brazil. The company identified a gap: healthcare providers depended on generic payment acquirers with no integration into clinical or financial workflows.
Amigo Pay was built as a vertical acquiring platform for healthcare. It embeds payment processing directly into clinical management software (Amigo Clinic and Amigo One), connecting credentialing, transaction capture via POS terminals, and settlement through a BaaS infrastructure.
Key Features
Integrated Acquiring
Payment processing embedded into clinical software. Clinics manage appointments, billing, and collections in one platform instead of juggling separate tools.
Multi-Channel Payments
Card-present (65%) and card-not-present (35%) transactions with support for debit, credit, and installment plans up to 12x, matching how healthcare payments actually work in Brazil.
Real-Time Financial Dashboard
Standalone dashboard for POS terminals plus financial modules inside Amigo Clinic (web) and Amigo One (mobile), with cash management, bank statement reconciliation, and AI-powered transaction matching.
Low-Friction Credentialing
End-to-end merchant onboarding with KYC, device provisioning through a remote device management platform, and on-site installation and training. Clinics go live in days, not weeks.
Tech Stack
- React – Frontend for the Amigo Clinic web platform, standalone POS dashboard, and merchant management portal.
- React Native – Amigo One mobile app for doctors. Agenda, prescriptions, accounting, and payments in a single native experience.
- Node.js – Backend services for transaction processing, webhook orchestration, and integration with the acquiring infrastructure APIs.
- TypeScript – Type safety across the full stack, critical for financial calculations and payment flow correctness.
- PostgreSQL – Primary data store for merchants, transactions, settlements, and audit trails with strong consistency guarantees.
- AWS – Cloud infrastructure supporting the payment processing pipeline, auto-scaling for transaction volume spikes.
Challenges & Learnings
Building a Vertical Acquirer from Zero
Amigo Tech had no payments infrastructure. Building an acquiring arm meant integrating with a BaaS provider for processing, a device management platform for terminal lifecycle, a POS provisioning service, and card brands (Visa, Mastercard, Elo, Amex), all while keeping the merchant experience simple.
The architecture had to support three distinct capture surfaces: the Amigo Clinic web app, the Amigo One mobile app, and standalone POS terminals, each with different integration patterns but a unified settlement pipeline.
Scaling Transaction Volume at Startup Speed
TPV grew from R$ 35k in January 2025 to R$ 100M+ by February 2026, reaching that milestone in 10 months. The system had to sustain exponential transaction growth while keeping chargeback rates at 0.00012% and maintaining settlement accuracy.
Installment payments dominate the healthcare vertical (76% of revenue), requiring complex split-settlement logic and receivables anticipation (40.4% of volume). This is fundamentally different from typical e-commerce payment patterns.
Outcome
Amigo Pay launched as a greenfield acquiring and banking platform in early 2025 and surpassed R$ 100M in TPV within 10 months, with acquiring revenue growing 154% and banking revenue growing 94% in the last quarter.
